I understand and agree that:

• The percentage of proceeds is determined solely by Jitty Joe's

• Only qualifying sales during the approved time period are included

• No onsite cash handling or outside fundraising is permitted

• Payment will be issued after the event is completed and reconciled (no less than one week from the date of the event)

• Requests must be submitted at least 30 days in advance

• Submission of this form does not guarantee approval
